Monitoring Template not returning data

I have configured a Monitoring template to return the failover status for a device. While the test process works when creating the template as it returns the required information, when assigned to a device, the returned data is always blank.

Is there something special required to create a custom monitoring template and have it work?

Could you confirm that the issue is still present with this monitoring template? There have been a lot of changes over the month regarding the monitoring templates.

If it is still present, could you confirm that the monitoring template itself is enabled under Actions → Management → Monitoring templates:
